How we work

We have listed the 3 simple steps, from enquiry to care, to help make everything as easy as possible for you.

Step 1: Call Us

Simply give us a call on 01844 447619 to discuss what you are looking for and how you would like us to help. Everyone here at Autumn Years Care is very experienced in what they do, so you can rest assured your phone call will be dealt with compassionately and professional.

Step 2: Care Asessment

Before we are able to enter your home and provide care, we will carry out what is known as a Care Assessment. This assessment is always done face to face. Ideally this step is carried out at your own home, however if you are currently admitted to Hospital we are happy to visit you there. This meeting gives you the chance to meet us and ask us any questions you might have. A Care Assessment is made up of two part's, a Client Assessment and a Home Assessment. The client assessment will focus on you, any medications you might be prescribed and also what you would like to get out of the service. The Home Assessment means we are able to ensure any possible trip hazards or other such areas which might cause you harm are highlighted to you. The aim here is to use our experience in helping keep you safe at home.

Step 3: Care Package Commences

Once the above two steps have been completed and an ideal start date has been picked by you, we will simply start the care package on that date. We will also use the information given during the Care Assessment to create your own unique Care Plan.

This Care Plan will be kept in your house during the period we are providing care for you. A Care Plan contains information about the company, a list of tasks and aims and many more useful pieces of information.

  • Why should we use Autumn Years Care?
  • Because everything here at Autumn Years Care is focused on providing only the very highest quality Home Care available. Because we put you, and your family first. Because we pride ourselves on only employing the very best Home Care Workers around have a high entry standard for anyone joining the company, ensuring you get the best level of care possible, given by what we think our the best carers in the industry. Because to us creating a few more smiles in the world every single day, by helping you or your loved one live independently at home is what we do best.
  • How do we arrange a care package with Autumn Years Care?
  • It's easy. All you need to do is give us a phone call on 01844 447026 and we will then discuss your exact care needs. From there we will arrange an assessment with yourselves, at a time which suits you, and then it’s just a simple case of picking the start date. All of the team here at Autumn Years Care are very experienced and will guide you through the process every step of the way.
  • Can I find out what others who used your service thought of it?
  • Most defiantly. Feedback is something we value very highly. We believe it’s key to us being able to constantly improve. Therefore we use a “Rate & Review” website called www.trustedcare.co.uk where you can see some of the positive feedback we have received so far.
  • How is Home Care Funded?
  • There are several different ways in which Home Care, just liking living in a Care Home can be funded. To simplify things the main two groups are Privately Funded or Social Care Funded. We specialise in providing Home Care to the Privately Funded Sector, this means that we are directly answerable to the client, which in turns means we have to provide only the best Home Care going. It's not to say we don't take on Socially Funded Clients, as we do, however unlike many other Providers out there we would rather focus our time on YOU than on keeping a contracts manager happy. We strongly feel this way of thinking plays a big part in why we pick up so many awards for the exceptional Home Care we offer.
  • My Mother has Dementia, is this something you can help with?
  • Dementia can be very challenging for both the person who has it but also those around them. We are very experienced in helping care for people with Dementia but also with providing support to those around them. Why not give us a call and see what we can do to help on 01844 447026.
  • What types of Care Services do you provide?
  • We provide a full range of services ranging from Companionship right through to End of Life Care. Why not have a read of Our Services section of the website to find out a bit more.
  • Are your Carers qualified?
  • All of our Carers are very experienced and handpicked by us. Our high standards mean that we turn away far more than we employ. In fact less than 8% of interviewees obtain a job within Autumn Years Care. Each Carer also goes through rigorous training consisting of many different Certificates/Qualifications (which are constantly updated) before being allowed to provide care to a client. In addition all Carers hold a valid DBS (Disclosure and Barring Service) certificate, formally known as a CRB (Criminal Records Bureau) check.
  • How many different Carers will visit me?
  • We understand the importance of having a friendly face providing your care and therefore we allocate a small and select team to each client we take on. This means you will get to know your Carers, which in turn means they can get to know you to.
  • We currently use a different Care Provider, are we able to switch over to you?
  • Of course you can, in fact we often look after clients who have previously used a different Care Provider but weren’t happy with the service they received so switched over to us. We will be there every step of the way in making the process as simple and straight forward for you as possible, giving you a smooth transition.
  • Why do you use the phrase “creating smiles daily”
  • This is because providing good quality Home Care is an important and serious job, however we also understand that as people we all enjoy to smile as well, therefore not only do we meet your needs, but we will also try to create a few more smiles in your daily life, by lifting your spirits and providing compassion.

We are Registered with the Care Quality Commission (CQC), which is the Industry Regulator. A part of this Regulation means that all care providers are required to undergo an Inspection. We are proud to have been rated “GOOD” at our first Inspection.
